"Urban Dictionary (s.v. Going Commando), a former infantry soldier and medic gives a plausible explanation. He writes that, when on the field, soldiers sweat a lot and can’t take showers for days. Their uniforms are loose enough to allow for ease of movement, and they don’t wear underpants in order to prevent skin eruptions and fungal infections."

I know not the answer to your question, but at least it gives me an excuse to say I really hope you're feeling better today.

Because of the tv show Friends. There’s an episode where they coin the phrase.
SomeMichGuy · M
@ChampagneOnIce Umm...no...much older than that.
ChampagneOnIce · 51-55, F
@SomeMichGuy Well, I hadn’t heard of it before that. It was used a lot after that, and it seemed to be because of that episode. 🤷🏻♀

The term “commando” refers to a type of military unit that operates in stealth and camouflage, not to do with undergarments. The phrase actually stems from the South African Defense Force’s elite unit, the “Recces,” who allegedly wore no underwear as a practical and stealthy measure during missions. The term eventually spread to describe anyone who chose to wear no underwear, but this was not its original intention.

Because obviously, those who go that way “commando” lot of attention. 😍

I always picture a Rambo character running thru the jungle when I hear that term.
