In the computer age with the prevalence of hackers i would be concerned about questions 2 and 3, middle name is often used as a password and birth date could make it possible to get credit cards in your name...especially if they figure out your last name somehow.

2 and 3 seem a bit early to be asking about after a few hours. 1 seems like an odd question unless height is important to who he might be attracted to.

At the end of the day, whether the questions are creepy or not if you feel uncomfortable answering them he should respect that was a response. If he's not letting that go, it definitely becomes creepy if it wasn't before.
