ear pain from being congested while the plane changes altitude fast, the pressure can sometimes pierce the eardrum. same thing with the sinus cavities in the plane, it can cause a fracture of the sinus cavity. I had both, at different times, no pierced eardrum or fractured sinus but holding my head with two hand screaming. also one time had a splitting headache of unknown origin which left me on my knees holding my head crying and screaming.
when i broke my tailbone. then, when i slipped while it was healing and broke it AGAIN

My eye fell to pieces on me from a super rare medical condition. After roughly 6 months of feeling like I had a red hot knife twisting in the socket all day they finally stopped trying to save it and just removed it. By comparison when I passed a 12mm kidney stone a few years later it was a distant second on the pain index.

Breaking my pelvis and having to sit on it and my also injured tailbone while being forced to allow it to heal badly with no medication whatsoever. Combined with frequent partial shoulder dislocations. And inflammation :/
physically natural childbirth....mentally there are things I still deal with
Hands down... Kidney stones are the worst, followed closely by the pain of a broken jaw.

horrible cramps during my period that makes me get super hot and then a cold chill alongside vomiting out both ends and then passing out and feeling all better. It used to happen a lot when I was younger but now, not so much.
