About that guy at the store, I believe he knows nothing about your episode at the bar nor you being trans. I think he is purely interested in you and is flirting with you, especially given the mischievous smile... That would be a good flirt tack tick I would employ. The meaning he is coming off with is "Oh, you have not changed your last name yet", as in you have not gotten married yet in the old world tradition of taking the guys last name, asking you this is a round about way of fishing for are you engaged or dating. Nothing about you being secretly trans. The guy is into you and likes your look.
