It's a disease, and quite a complex and intricate one too. My birth mother did a lot of drugs while she was pregnant, resulting in me being born not only in an unstable condition but also with a multitude of disorders. However it's definitely safe to say I was lucky in terms of what could have happened.
According to my doctor due to being exposed to it before I was even born, and my body going through withdrawals from it in my early years, addiction to each and every one of the drugs I was exposed to runs in my blood. Not only that it is essentially guaranteed that I'd get addicted again due to genetics accounting for a much higher percentage of my vulnerability to addiction. As a result it isn't safe for me to take certain drugs that I was exposed to for medical use.
It's definitely not a choice for me, I was born addicted to something I had no choice in partaking in in the first place. As a result it's a disease that gave me a bunch of mental disorders I didn't ask for and it's a disease that exists inside my body and has made my life significantly harder.
The addiction itself is no choice. risking it may be a choice for some people, but even then not always

Both. Addiction is a symptom of something else, either mental illness or physical illness, and it isn't always a choice. It should still be treated as a disease regardless of why it began though
