It is almost impossible to apportion blame in this regard, but at the same time, it may be possible to define general responsibilities and goals. Accountability must be understood as requiring individuals and organizations to take actions to ameliorate at least a portion of the harm they contributed to, but it also needs to include the option of actions that genuinely contribute to solving the problem. And a big issue is determining what those actions actually are.
