If there was an illness that only afflicted a small portion of the population...

...and this affliction seriously affected the perceptions of those it infected so much so that the perceptions of the afflicted are that the world is made up of two types of people, themselves and the "nonpeople". The afflicted demand that the rest of us call ourselves "nonpeople" and conform to their perceptions of us.

Would the rest of the population be required to humor and honor their perceptions?

Or would the onus fall to the afflicted to get better and view the world as they did previously?

When you cut out a sense of a higher power everything gets murky or at least a higher moral law.
That or you could try to depend on collected data that shows one way of thinking produces mental wellness and the other brings a sickness.
But ,then you might have a trouble with definitions of what constitutes " good' outcome and what is "bad' outcome.
The" ill " may think the "well " are the "ill "ones and start to convince the so-called "well "people that they are the ones that are in error.
How would you come to the conclusion that there really is some kind of illness between two different sets of people both claiming to be "well"?
Unless ,you believe in a moral law or law giver, outside the opinions of people,then everything is perception or relative to each individual.
And perceptions by nature are not provable unless there is a standard that one can measure against the perception.

We are pretty much living in that type of state right now. I believe the smaller population need a thorough round of penicillin for their infection.

I'm standing beside the majority and telling the afflicted to STFU and take their meds. No I have no empathy when I'm expected to conform for them.
I think the onus would be on the afflicted person, would it be fair to force everyone acknowledge and normalise person’s internal subjective state?

However there are ethics. What if the person seeing everyone as non-people doesn’t realise that it’s not everyone’s default setting?
It has happened much like this. HIV. COVID 19.

Eventually the world moved on as normal, but it could be hell for the afflicted.

Rumor is more devastating than disease, but when we know little about the disease, rumor leads the way.
