I believe it is toddlers who are bored and throw them out. They have the windows open maybe due to not having A/C and they just find just to throw out while their parent is distracted by driving. I used to live in a rural area and would see shoes and school lunches on the side of the road. Kids on school buses do it to other kids. One time I saw a shoe and then about a quarter mile down the road there was the mate to it.
@Adrift All it takes is you open your car door and it falls out and you don't notice it, or maybe the dog barfs in it or something so you toss it.

@Adrift Probably someone on a long road trip who took off their shoes to be more comfortable. Then stopped for whatever reason, and the shoe fell out when opening the door. If it's a right shoe probably a passenger's, If left shoe more likely the driver's. I would imagine that you would see more right shoes on the side of the road. Left shoe when in the UK, Australia or New Zealand.
