I was taught that it was a term of respect, so I choose to take it that way. I hear it a lot since I'm old.

I have heard ‘Don’t call me sir I work for a living’ I think American children are often brought up to use it to be polite.

I don't think anyone has ever meant "sir" respectfully. It's an insult.
Depends on how it's used. In general it's a term showing respect and/or recognition of authority.
