Do you ever have apps just disappear off your phone?
All of a sudden, there will be a blank space on the home page where an app used to be, and I can't even remember what used to be there until I go to use it.

Nope it should not happen, unless those were factory installed apps that may get removed when you update the phone software.
If it is android, you can go to app store and check the history.
Also scan your phone for viruses.
If someone else uses your phone, you might want to check if they removed something
