He probably fears that you'll treat him differently. It's very common for people with medical conditions, illnesses, and disabilities to be treated as stupid, untrustworthy, unreliable, or even weird.

He could be uncomfortable with sharing his condition just yet. There is a lot of shame and fear that people face regarding their medical histories. Not that there should be any, we're all human and prone to illness.

Maybe he's ashamed of the condition. Just be his friend, and that may be the therapy that he needs to one day tell you.

Could be herpes or another std or maybe a disease where he can’t control his bladder or bowels.
