Get a pair of pliers or better yet channel lock pliers and use it on the cap. It will bite and turn with very little pressure. You don't need to worry about breaking the bottle. Just use enough grip so it doesn't slip.

Run the top of the bottle under some very hot water for about two minutes. Dry your hands completely, and try again.
