Yeah that’s not cool at all. I had a similar friend who would only call to “catch up” so they can ask if I’d be a reference for them.

Next time you see her turn your back on her and act like she doesn't even exist. If she ever says something to you. Say I don't like people who use other people so so I chose to " leave you alone" and then just walk away

Don't fill out the form. If they ask why, just say that circumstances have changed, not for the good, or bad, just changed and you just want to remain neutral.
