Don't keep anything from them be totally open and honest regardless of how awkward and/or embarrassing it may seem to you. It won't be easy, I won't lie to you, but it's something you have to do if they are to help you.

Good for you, that takes courage. Normal to be scared and embarrassed. Just try your best to stay calm and don't hold anything back. You could get better help if the doctor knows everything. I hope it goes well for you, it will probably take a long time to notice any change, but this is the first step. Don't get discouraged.
