Alright, let's start with the origins. The terms First, Second, and Third World, at the time of their conception, referred to nations allied with NATO, the Eastern Bloc, or neither, respectively. So to say that America is "the only First World country" would mean that the entire rest of NATO does not exist.

Since the end of the Cold War and the dissolution of the Soviet Union, the terms have been repurposed to refer to how modernized and developed a country is. In which case, America would be at best Second World, perhaps even Third World when you get outside the urbanized areas.

No matter how you slice it, America is not, and has never been, "the only First World country." You just made that up right now.

She went on to praise cigarettes, saying that it's good to smoke tobacco because it's a stimulant, you'll work harder than then die at age 50 and be less of a burden to taxpayers.
