Not in the US I don't think it is.. except for famous people. I only know of one person in my real life that has had it.

Popular and affordable are two different things. I know a great many Americans that would do it if they had the money. It's also a bit regional. Ex. Its far more popular in California than in Georgia.
Right.. wanting to do it and doing it are two different things as well.
