Does it piss you off when people say how "fat" they are...
...when they're in need of a few compliments? I just ignore people who say that, they know they're thin, as does everyone else.

There are many reasons that people do this... insecurity... body dysmorphia etc. I agree some are seeking attention, but many have an illness and they genuinely think they are fat when they aren't.
