if theyre suffering from severe depression and/or anxiety or if s/he just needs a vacation?

well if you take the vacation and it doesn't improve - you may have one or the other.

anxiety and depression hold hands - sometimes taking an antidepressant will help with the anxiety

at least in my experience.
