SARS-CoV-2 causes the immune system to flood the respiratory tract and lungs to fight it off. Sadly for some this response goes into overdrive causing viral pneumonia in a way we've never really seen and people simply can't get oxygen into their bloodstream.

Hiv is actually much more like the plague in that it directly attacks the immune system via the ccr5 receptor.

Interestingly that is why aids is less prevalent in Caucasian populations than esp sub Saharan Africans. Due to the plague wiping out huge amount of the population in the 1300s many who survived had a mutated ccr5 receptor so hiv can't infect them. It simply can't latch on. So Caucasian populations have a high prevalence of that mutation, as many without that mutation died out.
