unable to log out nor delete a facebook page

I am in twice. Two different names. The one I want to keep has my middle initial. I have struggled, gone through all the clicks to deactivate the original, and the it still remains. Rebooting as well and it still is there. Has anyone else been able to deactivate or remove a name. Yes I have the pass word. I cannot kill it. Am I stuck with two forever? I have all 4 steps steps written down. Can't reach F.B. to do this for me.

Thankyou, I will try. The first time I put in a Face Book name I simply hurried along. But soon after I decided I wanted to add my middle initial. I do not want there to be more than one Carla.
