Last week, I was asked by an HR rep to send in my 5 references and have them do a survey regarding me, my work ethic , skills etc.

One reference has fell through, telling me that she hasn't had a chance to look at it.

It's been 1 week. That's more than enough time.
I have already asked/ reminded her to do it .

** I do not have anyone else that I can ask for a reference.

Do you think it'd be okay if I use myself and vouch for my own skills, abilities, work ethic etc?

Or do you think that would automatically disqualify me?

***EDIT****
I am NOT trying to be discrete
and pretend to be someone else. I will use my name and contact info, and speak in first person about myself and why I think I'm right for the job

Resist the temptation.

@Iwillwait please read the above edit. My goal is NOT to lie nor be discrete and pretend to be someone else

@iamnikki I get it, and I totally understand you wanting to be able to be a reference, but that ultimately is a conflict of interest (if you can believe it) lol! It is. The 5 references need to be anyone but you. That is where the validity and gravity of their testimony about you and your work performance gains traction with the Hiring Manager.

@Iwillwait but I have submitted the 5 references, just as they asked. The system even reflects that I did and that they didn't complete the reference, so I'll be number 6...still a bad idea?

@iamnikki no, if you the 5 they asked for, there's no issue being a final last word, so to speak.
