I can't decide which one...
Because on one hand they made a whole different person, but on the other hand that's a really jacked up looking new person.

They did not make a whole new person.
His chromosomes, blood type, DNA and etc are still all the same- they took off and put on.

Yep, similar to an Appendectomy but this operation is called TakeMyDicFromMe (not sure if the final stage has been completed). No amount of swapping junk can change the DNA from XY to XX. Besides you know what DNA stands for - Do Not Ask.
