It does matter if they marry, or at least live at the same address and she becomes a substitute for the mother.
Hence the name "step"-mother. The title makes a lot more sense than "biological mother" because that is tautology based on the obvious: everyone can only have one father and mother so anyone taking either's place can only be a "step-parent"
