Yes, quite few times, never on purpose.
Yes and I still keep sadness and dissapointment about.
In all those cases I did what I thought it was my best BUT I overestimated my capacity to overcome problems that I obviously thought were hard but not impossible to solve.
In one of those cases I was also probably biased for some kind of ethical dilemma.

Yes I have, many years ago though. I learned a lesson from that, I now never make promises. Now I say, yes I will, unless a very good reason for not doing it happens.
