Well. I agreed to marry a guy, but it didn't work out and I ended the engagement. So I guess that was a broken promise.

Yes, quite few times, never on purpose.
Yes and I still keep sadness and dissapointment about.
In all those cases I did what I thought it was my best BUT I overestimated my capacity to overcome problems that I obviously thought were hard but not impossible to solve.
In one of those cases I was also probably biased for some kind of ethical dilemma.
