Epididymal hypertension (aka blue balls) is very real yet not very common. Most males secrete semen and sperm all the time in small amounts (wet dreams). Yet when there is a blockage it can be quite painful. The degree of pain is dependent on the blockage.
The blockage can result from many things. Even psychological reasons... hence the hypertension.
