Also how often to people mistakenly think they deserve an apology at all, when that's the furthest thing from your mind with them, and you simply laugh at the notion?

Not often. I’m the one always saying sorry all the time , sometimes when it’s not even needed. I’m a people pleaser but I’ve been changing that.
