That image appears to be by artist Christopher Lovell for The Exorcist III promotional materials, although this work is more recent that the movie.

It appears to be Jason Miller, who played Father Karras in the original movie and is listed as "Patient X" in the third.

@Wol62 No, he was not creepy, I think there was a commercial where he appeared for a few seconds, but I always wondered why his nose was like this and why his eyes were yellow.
