@NerdyPotato didn't think people would take it in this way. I didn't want to add the phone number and email address options so that people can fill the form without hesitation but our HOD asked us to add those.

@RileyWinters it's fine if that's necessary, but maybe search for participants in shopping malls or something. Nobody on SW is going to provide their full name, phone number and address.
