My first impression is because manfriend is more of a husband thing...
Whereas boyfriend is the early stages of a relationship prior to marriage...

After research & investigation on the etymology of the idiom in question, I discovered the Institute of Romantic Affairs and Nomenclature determined this to be exactly correct.
