A transgender person may become transsexual, meaning that they get operations and hormones to help them become the gender they identify with.
There are operations to get breast implants and genital changes. A vagina can be created while I think penises are currently prosthetics, not lab-grown or something.
Also, I think you may be confused on what transgender means. For example, a transman has a masculinized brain (in shape and function). A transwoman has a feminized brain. The cause of transgenderism is unknown but has evidence of starting in the womb. The metaphor is actually pretty succinct: trapped in the wrong body.
There have also been many studies you can look into, such as psychological ones, including the unethical case by Dr. Money, and neurological studies- examinations of brains through scans, as well as autopsies.
