Sleeping pills help. Day before the appointment choose the time you want to sleep, perhaps fairly early so you can be prepared for the next day. Try to take 20mg worth of melatonin, with a sleep aid such as NyQuil.
Upon waking up the next day, caffeinate yourself and eat breakfast so you can wake up from all the groggy side effects.
Then you won’t have to worry about taking pills again until another impromptu appointment.

Not good. I had a time before my wife passed when as a caregiver, I was stressed and full of anxiety. All that equals no sleep. After she passed, I also dealt with it for a few months.
Now, I hit the sack between 9-10 pm. I fall asleep within 30 minutes. About every other day, I take 10mg of Indica cannabis gummies. Works like a charm when needed. But generally by changing my routine, my body has adjusted and get 6-8 hours per night routinely. Dramatically increasing my exercise routine was a big help as well.

I went through that for months after Vickie passed. However I'm still on the routine we had together after I retired. It very well could be the appointment itself troubling my subconscious. It's just a question and answer visit but it's all about what I can and can't do for myself and do I take my meds, stairs, laundry, groceries, etc. the thing is many of these things don't apply. I don't go to stores I order delivery. I live mostly off Amazon because of my diet. Many of the questions I know are coming can't be yes or no answers and that drives me nuts.

Hope you get some sleep soon.
I'm in bed by 10 usually and up at 6:00 am.
We have our granddaughter here, because her dad works nights,so up for school runs.

Try not to crash during the day, and hang in there until 9 or 10 p.m.
Then get a good night's sleep and set your alarm for, say, 7 a.m. to get back to a normal schedule by Wednesday.

@Thinkerbell That's not my normal schedule. I sleep from 3am to anywhere from 1 to 3pm.
I don't have a problem waking early for appointments however. My appointment Wednesday is 3pm and it's only 8min drive. If I get up at 1 or 2 all will be normal. But I am going to try to make it through the day without crashing. That way I will sleep tonight probably exhausted but so be it.
