I would be cautious in any situation where you assume to know the motivation behind a persons actions. I have a strong feeling you are reading too much into something that just isnt there. Its kind of concerning actually

What has her cough got to do with you not knowing her?

Ask if she is okay or needs a drink of water.

Maybe that's her way of trying to get you to notice her and introduce yourself.
