Be sweet about it! Men are wired differently, they don't remember these things. Remind him before, like two weeks before or so. Tell him clear you'd like a nice date, something with dressing up and going to dinner or that you'd like a night in and be cuddly and comfi. Be clear in that you want and then the ball is in his court. Make a back-up plan, something to do that you can afort to do with the two of you and have that in case he is too busy or cannot find any ideas. There is no shame in a woman planning a romantic date for her and her love.
