Go online. They have examples. I know, right? I had to do this a few years back. Horrible. Had to dig through pspers stored away.Good luck. Only make it one page. Do not include your age. Just your last ten years. When you get to an interview, you can dazzle them. Ageism exists out there. Sadly.

:( You bring up a sharp, bittersweet memory of what I went through 40+ years ago. I once worked for a collapsing airline, in a seniority based industry. Made a complete career change in my early 40s while dealing with other personal tragedies.
Besides all of that, there are kids in school, mortgages to pay, etc., etc., etc. and the options aren't exactly crystal clear.
If I can offer a bit of advice, don't hesitate to go back to school or take other steps to best prepare yourself for a transition if you feel that you have to abandon the old and start over.
