How does this happen?

Not enough ammonia

seen it done,,put the nozzle in tank,, go in and get food, and drink,,pay for gas food drink,,go get in car drive away,,,,,oooos i forgot the fu** ing nozzle,

same as the drive in movies when they had speakers,,rips out car window

there's one anwer right there, Russiaz the land where people are So drunk you Can't get car insurance without a dashcam

They didn't check their mirrors or double check before they tried to leave. And their gas lid is on the passenger side which makes it easier to forget.
