Can I send a copy of my personal injury form ?

Apparently my insurance is saying they never received my PIP form for my car accident. A personal injury claim form. It consists of four forms. I had made a copy of them once completed, and I'm wondering instead of filling out all four forms again, is it ok to send a copy? And then just write my signature on each again , under the copied signature?
