In most functioning democracies, the "people behind the curtain" tend to be very powerful because elected officials tend not to have expertise in the day to day matters of running state, while their staff do. There is a danger that staffers are given too much power because they have a tendency to kill major changes as being too risky.

There's also the fact that world leaders tend to be accountable to a caucus or political party. In a parliamentary system like Canada or the UK, the parliament runs government NOT the leader, who carries forth the will of parliament. It usually ends up leaders are more powerful than intended by the system.

In North Korea? I'm pretty sure what Kim Jong Un wants, he gets.
