Can you always tell the difference between constructive criticism and criticism that is primarily intended to wound you?

It's a direct attack on your character or the way you do things.

Lol the hidden intent in constructive criticism is to upset you, shatter your illusions, your arrogance, your empty pride.
The person giving constructive criticism itches like an OCD patient to let out what they really think about you. They're usually angry. They take pleasure "in opening your eyes".
If someone wants to give you constructive criticism without harming you, they choose their words carefully. That's rare.
