Where I live, some people use the words "how are you?" as a general greeting instead of hello, good morning, et cetera.
They aren't expecting a response that answers the question. It's not that they're being rude, it's just a very peculiar colloquialism.
That might not be the case where you are but I thought it was worth mentioning.

@SaorUladh i know what how are you means. I also understand its just a thing to do to be nice. But we are friends and i havent talked to her in a while. And we both have physical issues sooo i really wanted to answer the question without her being that way.

@TurtlePink In the UK too. When you meet somebody it's normal for older people to shake hands and say "how do you do?" This is not actually a question or an enquiry as to their wellbeing
