Practically all the time to be honest...The only time I wear underwear is when I have a doctor's appointment, have to try on clothes, some jobs I have held, or have to wear more formal style clothing (slacks without underwear just doesn't work well). I just find it very uncomfortable most of the time.

For me it's always commando. After a long time without wearing underwear, I don't think I could stand the feeling of something underneath now.
