If i write a non fiction novel that mostly included someone and their mental issues but i change the names to protect them... can i still get sued?

If you do the whole "This is a work of fiction and similarities to people are purely coincidential" then it's doubtful.

not really - but you need to have the consent from them clearly mentioning to them that their personal details won't be revealed in anyway

I don't know, i assume not because how would anyone know it's about them?
i agree its about someone i care about and their mental deterioration and .... a dream i had thats all mixed in it. i need to do this. i want to be a writer and this idea has been calling my name. i just outlined the first five chapters.

@RoboChloe: no his family doesnt like me because i'm black... and it ended badly to where his probation officer had to get in to it. so theres definately no way for me to even ask... however the story line is going to be very good.
