Does reverse image search always work?

There's a handful of girls on here that were using fake pics.. and portraying it to be them. One admitted to it but I think the others blocked me lol! 😆

Nope, and you can usually fool it because the algorithms don't really account for tinkering, cropping, flipping, recolouring.
