it's a nuanced thing. it's an indirect insult that you say to someone about someone else, usually. "well, bless their heart", usually with an accompanying snarky look. it usually means someone is lacking in intelligence

but it can also be said as an insult directly to someone's face who [i]doesn't get it[/i], as the person who said it looks around at others for confirmation on what was meant, or to a friend to gently rub something stupid they said or did in their face a bit.

it can also be said in earnest to someone, directly, by someone who genuinely feels for a person's situation/problem/woe. it's usually apparent due to the nature of the exchange and the inflection in their voice. as well, it can be a way to say [i]thank you[/i] when someone does something kind that was unexpected

@XenonRush only jokingly when mocking southerners. i happen to live south of Interstate 10, which is a bit less southern (i know, weird) in culture due to tourism and military bases, but i have been up in the sticks where people, usually older females, say it regularly
