it's a nuanced thing. it's an indirect insult that you say to someone about someone else, usually. "well, bless their heart", usually with an accompanying snarky look. it usually means someone is lacking in intelligence
but it can also be said as an insult directly to someone's face who [i]doesn't get it[/i], as the person who said it looks around at others for confirmation on what was meant, or to a friend to gently rub something stupid they said or did in their face a bit.
it can also be said in earnest to someone, directly, by someone who genuinely feels for a person's situation/problem/woe. it's usually apparent due to the nature of the exchange and the inflection in their voice. as well, it can be a way to say [i]thank you[/i] when someone does something kind that was unexpected

Now I have two things the hammer the boss with tomorrow.

And I will also accuse him of being toxically positive.
These are two things I have learned from SW today alone.
Oh and by the way I spent a lot of time in the south when I was younger and I never heard anyone say it nor had I ever said it myself.
