Does it include the right to continuously shout so as to prevent others from speaking?

The first amendment freedom of speech only technically protects US Citizens from retaliation from the government when criticizing politicians and the government in a peaceful non malicious way, but over the ages the true intent has been perverted through US Supreme Court rulings.
