These criminals succeed because...
a) organisations do not protect their IT systems sufficiently deeply and rely far too much on common software, trust in others' IT security, and physical connections;
and b) enough victims pay enough to make it worth the attackers' while.
An entire city can only fail if every department and business in it is fully interconnected and via the open Internet, with no "firewalls".
