I vaguely remember a strange movie scene and I was hoping someone could tell me what movie that was

I saw this in about 2005 so the movie would have had to come around that time or before then it was just this guy sitting in a bathtub and this old-fashioned-looking nurse walks in and the guy starts screaming and she is holding something she puts it...
