I vaguely remember a strange movie scene and I was hoping someone could tell me what movie that was

I saw this in about 2005 so the movie would have had to come around that time or before then it was just this guy sitting in a bathtub and this old-fashioned-looking nurse walks in and the guy starts screaming and she is holding something she puts it on his chest and I'm pretty sure he sank to the bottom if the tub I'm pretty sure more people walked in with her but she's the only one I can remember
