You are resilient, you know your weaknesses, but you also know your strengths and you have motivation to advance - you can conquer this phase in life.
Tips from my own experience:
Body&Mind:
Try to allow yourself some time to wind off - if you are disconnected from yourself, your mood and judgment will also become chaotic.
If you have some time, as little as it can be, spend some time outdoors in nature (hiking) or go to a yoga (or any moving-related) class to release that tension. You will see that the more you move the easier it will become to regain focus.
Nutrition:
Also.. one thing that might help is diminishing fabric sugar and salt to a minimum and focusing on fruit or dry fruit and a little dark chocolate or honey in small amounts if you need a sugary lift - anything milk, sugar, salt related seems to cause me to be more confused and forget more things. When I eat cleaner and move (walk instead of car for example), it helps me to clear my mind and be more organized in my thoughts - it helps!
Soul:
You may feel closed. Closed indoors, in a cage that was built by life to keep you running in one place. Open the door and get yourself freedom. Anything art, social, or intuition-related will ease your emotional suffering. You do not need to have a thousand friends to be content. Even 1 is enough. If you are alone and do not have friends yet - get yourself out a bit more often and treat yourself to your city's events, even by yourself - you never know where you will meet your next best friend. You can also sign up for a weird class you never thought you'd take like cooking, gardening, sewing/knitting, robotics, meditation, languages, crafts (drawing-painting-sculpture-textile-jewelry-so many hand crafts to learn!) - sometimes you may just need a totally different environment to refresh your brain and feel better.
I am saying all these methods from my self experience and I am living on perpetual burnout to burnout because life is so demanding BUT those little tricks help me keep my feet stable and step forward regardless of the surcharge. When your tank is empty and you still find strength to step up - it means you have disciplined yourself to get yourself back up every time - and you will feel energy that you did not know you had - from your inner drive :)
